Medication Forms

  • Medication may only be administered by designated Renfrew staff with written consent from a parent/guardian.
  • All medication must be in the original container and handed directly from the primary care giver to a Renfrew staff - teacher, CDF or bus driver -  NOT carried by the child in a bag or backpack.
  • Medication will be tagged and put in locked storage out of the reach of children.
  • All medication on the premises is kept in locked storage.
  • A child carrying daily medications, including inhalers and epi-pens, in their backpack will have their backpack identified with a plastic tag with their name attached to their backpack. Tagged backpacks are kept at the front of the bus.
  • Only designated staff will administer medication.

Please print and complete the appropriate form and send or bring it to the school so that medication can be administered.

  1. MEDICATION Consent LONG TERM

    To be used when medication to be administered is of a duration longer than two weeks or is ongoing (such as an epi-pen).
